A fermentation-derived S-equol, standardized for consistency
S-equol is a gut-derived metabolite of daidzein, a soy isoflavone. In nature, only specific intestinal bacteria can convert daidzein into S-equol — a trait present in an estimated 50–60% of Asian populations but just 20–30% of Western populations. Individuals lacking this microbiome profile ("non-producers") see little to no benefit from standard soy isoflavone intake.

Selective ERβ signaling, not broad estrogenic activity
S-equol acts as a high-affinity, selective agonist of estrogen receptor beta (ERβ) — binding at roughly 20% the affinity of 17β-estradiol, compared to less than 1% affinity for estrogen receptor alpha (ERα), the receptor subtype predominant in breast and uterine tissue. This ERβ selectivity is the basis for S-equol's tissue-targeted activity in bone, skin, vascular, and neuroendocrine tissue.

Hot Flash Frequency
12-week randomized, placebo-controlled trial in non-equol-producing postmenopausal women: S-equol reduced hot flash frequency by 58.7%, vs. 34.5% with placebo.
Bone Resorption Marker
12-month RCT, 10 mg/day S-equol in postmenopausal women: 23.9% reduction in urinary deoxypyridinoline (DPD), a validated bone-resorption marker, vs. 2.9% with placebo. Whole-body BMD loss was also attenuated.
Vascular Function
A six-month randomized controlled study protocol is evaluating S-equol's effect on blood pressure and vascular function in equol non-producers with prehypertension or untreated Stage 1 hypertension.
